參賽聲明Personal Declaration

本人已詳讀「2025中國信託‧台北101公益垂直馬拉松」之活動訊息，同意且保證遵守主辦單位於活動訊息所載事項，保證本人身心健康，無已在疾病及既往症，無不適合激烈運動之疾病（包含且不限高血壓、心血管疾病、糖尿病、氣喘等，或曾經有不明原因的胸部不適（如胸痛、胸悶））及狀態；亦瞭解本活動所需承受之風險，自願參加辦理報名。競賽中若發生任何意外事件，本人及親屬願意承擔所發生之賽事意外風險責任，因個人疾病因素或故意行為所引起之事故，願自行負責；亦同意對於非主辦、承辦單位所直接造成之傷害、死亡或其他任何形式的損失，放棄提出任何形式之賠償、慰問金請求。自行評估投保適當保險，並同意台北101於公共意外責任保險承保範圍內協助處理理賠事宜，惟本人已獲得理賠項目、或因特別不保事項、或非屬理賠範圍等事由，不得再向指導單位、主辦單位、承辦單位及贊助單位等要求任何形式之賠償。
本人同意獨家授權台北101及其指定之單位，得於本次活動期間為本人之錄影、攝影，並得於辦理本次活動目的範圍?，蒐集、處理、利用個人資料保護法所列之所有本人個人資料。亦同意獨家授權台北101及其指定之單位，將前述著作及成績於全球、永久為重製、公開播送、公開上映、公開傳輸、改作、散布、公開展示、發行、公開發表。本人無償同意其姓名、肖像、聲音、號碼布號碼或成績，由主辦單位或主辦單位授權之單位（如賽務辦理之協力者）用於企業、賽事活動之使用、收益，無需另行取得授權。
非經本人同意者（非上述授權之對象），禁止於本次活動期間為本人之錄影、攝影，或用以從事任何商業行為。為避免影響賽事進行，非經台北101事前書面同意，活動時本人不得於競賽區域（包括但不限於起跑區、賽道等）進行網路直播、視訊通話、自拍，或其他對自己或其他參賽者為攝影、錄影之行為。如有違反，工作人員有權取消本人之比賽資格。
本人保證提供有效身分證明文件和資料用於核實本人身分，確認同意以上聲明並願意承擔相應的法律責任；一經報名後不以任何理由質疑活動訊息或競賽內容所執行之事項。若比賽當天現場初次體檢不合格時，同意配合進行複檢；複檢如仍未達標準，本人為考量自身安全，同意依醫師建議放棄參賽資格，不得要求退回報名費或由他人遞補。本人業經審慎考慮後決定參加。
本人如未滿十八歲，確認已由法定代理人閱讀、瞭解並同意本聲明之所有內容後，始進行報名；並於填寫報名資料前，本人與法定代理人均已閱讀並同意完整參賽聲明及所有規定事項。

I have carefully reviewed all the rules and regulations for participating in "2025 CTBC‧TAIPEI 101 RUN UP" and agree to fully comply with and abide by these rules and regulations. Furthermore, I confirm that our physical and mental health am in good condition to participate in this event, with no existing or past illnesses, and no illnesses that would be unsuitable for strenuous exercise (including but not limited to high blood pressure, cardiovascular disease, diabetes, asthma, etc., or those who had experienced unexplained chest discomfort (e.g. chest pain or chest tightness)), and I am fully aware of and understand the dangers and risks associated with participating in this race event and I voluntarily consent to participate in and begin registration for this race event. In addition, in the event of any accident or incident occurring for unforeseen reasons during the course of this race event, I and my family members, agree to shoulder full responsibility for any personal risks related to participating in this race event. Furthermore, I am willing to take full responsibility for accidents caused by personal illness or intentional behavior and also agree to waive any request for compensation or sympathy payment for injuries, deaths or other losses of any kind that are not directly caused by the organizer or co-organizer. I agree to purchase additional insurance if necessary, and agree that the organizer will assist in handling claims within the coverage of general liability insurance and to waive any further claims for any damages, harmful injury, death, or other types of damage or loss resulting from any such accident or incident, as well as not making any further claims against the supervising organization, organizers, and sponsors of this race event in any form of compensation resulting from such an accident or incident.
I hereby agree to exclusively authorize TAIPEI 101 and individuals appointed by the organizer to capture audiovisual recordings and photographs of myself during this event. Furthermore, within the scope of organizing this event, I consent to the collection, processing, and utilization of all personal data pertaining to myself as stipulated by Personal Data Protection Act. I also agree to exclusively authorize TAIPEI 101 and individuals appointed by the organizer to has the right to globally and permanently reproduce, broadcast, screen, transmit, modify, distribute, publicly display, publish, and make these works available. I agree, without compensation, to allow the organizer or authorized third parties (such as event collaborators) to use my name, likeness, voice, bib number, or results for business and promotional purposes, including event-related activities, without needing additional permission.
Without my consent (excluding the aforementioned authorized parties), any recording, photography, or use of such materials for commercial purposes of me during the event is strictly prohibited. In avoidance of disturbing or affecting the progress of the event, without the prior written consent of TAIPEI 101, I am fully aware that I am not allowed to conduct live broadcasting, make video calls, take selfies, or engage in other acts of photographing or recording myself or other participants in the race area (including but not limited to waiting area before start line, race track, etc.). In case of violation, the staff retains the right to disqualify me from the competition.
I guarantee to provide valid identification documents and information for identity verification. I confirm my agreement with the above statement and am willing to bear the corresponding legal responsibilities; once registered, I will not question the event information or competition-related matters for any reason. On the race day, if I fail the initial on-site physical examination, I agree to undergo a re-examination; if the re-examination fails to meet the required standards, I agree to be disqualified from the race following the doctor's advice for the sake of my own safety, and I am fully aware that the I will not be entitled to a refund of the registration fee or to have other person as substitutes. I have decided to participate in the race after careful consideration.
If I am under the age of 18, I confirm that my legal guardian have reviewed, understood, and agreed to all the contents of this statement before registering; and before filling in the registration information, both I and my legal guardian have reviewed and agreed to the complete statement and all stipulated matters.
