參賽聲明Participation statement

本人或本團體已詳細閱讀過本活動之競賽規程且同意亦保證遵守大會於競賽規程中所約定之事項，保證本人或本團體隊員身心健康，亦了解馬拉松/路跑比賽所需承受之風險，自願參加比賽方開始報名，競賽中若發生任何意外事件，本人或本團體及家屬願意承擔比賽期間所發生之個人意外風險責任，亦同意對於非主辦方所造成的傷害、死亡或其他任何形式的損失放棄提出任何形式的賠償索求，一切責任與主辦單位無關；亦明白此項比賽的錄影、相片、姓名、號碼布號碼及成績於世界各地播放或展出與販售及刊登於本會網站上，本人或本團體亦同意主辦或被主辦單位授權之單位得蒐集、處理及利用本人所提供之個人資料，及寄送相關路跑活動訊息或使用本人或本團體肖像及成績於宣傳活動上。本人或本團體同意獨家授權主辦單位及主辦單位指定之人，得於本次活動期間拍攝本人之影音、照片，並得於辦理本次活動目的範圍内，蒐集、處理、利用個人資料。亦同意獨家授權主辦單位及主辦單位指定之人，得公開、利用、販售本次活動期間本人之影音、照片。且非經本人或本團體同意者(非上述授權之對象)，禁止於本次活動期間拍攝本人之影音、照片後，用以販售或從事任何商業行為。且保證提供有效的身分證和資料用於核實本人或本團體身份，對以上論述予以確認並願意承擔相應的法律責任。一經報名後將不得以任何理由質疑競賽內容所列之事項。選手如曾經有不明原因的胸部不適（胸痛、胸悶），不明原因的呼吸困難，家族心臟病、糖尿病史等皆屬心肌梗塞高危險群，建議選手慎重考慮自身安全勿勉強參賽或請醫師評估參賽風險及先接受心電圖檢測並自行加保個人人身意外保險。報名時請確認具有能力在限時內完賽，已完成報名手續者，不得以任何理由要求更換人名、參賽項目、衣服尺寸或退費亦不得因無法參賽轉讓該報名資格。無完成報名者將不具參賽資格，嚴禁陪跑。
=個資條款=中華民國路跑協會(以下簡稱本協會)為落實個人資料之保護，茲依據個人資料保護法（以下稱個資法）第8條規定告知下列事項：
依個人資料保護法等相關法令規定，在消費者、客戶管理與服務、契約及類似契約或其他法律關係事務、行銷、其他經營合於組織章程所定業務及為辦路跑活動及宣傳等相關事宜蒐集您與他人的姓名、身分證字號、生日、住址、性別、國籍、電話、電子郵件、緊急聯絡人及其地址、電話等；針對前述個人資料，本協會只會在由本協會擔任主辦/協辦/承辦活動的範圍及期間內在中華民國境內及主辦單位資訊儲存設備所在地處理或利用。本協會蒐集的個人資料，原則上僅會以電子檔或紙本形式供本協會、委外廠商或與本協會具有合作、委任等關係之第三人處理及利用。本協會保有您所提供的個人資料，依據個資法第3條規定，您可以對您本人的個人資料行使下述的權利：
(1) 查詢或請求閱覽您的個人資料。
(2) 請求製作給您的個人資料複製本。
(3) 請求補充或更正您的個人資料。
(4) 請求停止蒐集、處理或利用您的個人資料。
(5) 請求刪除您的個人資料。您欲行使上述提及之相關權利時，本協會服務專線為02-25855659。
本協會基於上述原因而需蒐集、處理或利用您的個人資料時，您可以自由選擇是否提供您的個人資料。若您選擇不提供個人資料或是提供不完全時，將可能導致無法成功報名相關馬拉松及路跑活動，或無法就本活動進行投保。
另您因活動所需，有蒐集選手或相關人之個人資料並提供予本協會之必要時，請依照本告知事項內容，向當事人充分說明並告知關於其個人資料保護的相關權益。

Participation Statement
I, or all members of the participating team, hereby declare that I/we have carefully read the Competition Regulations for this Event and agree to and guarantee compliance with all matters stipulated therein. I/we further warrant that I/we are in good physical and mental condition and understand the risks involved in participating in marathon and road running events. By registering for this Event, I/we voluntarily agree to participate. In the event of any accident or incident occurring during the Event, I/we and my/our family members agree to assume responsibility for the personal risks arising during the Event. I/we further agree to waive any and all claims for compensation in any form whatsoever for any injury, death, or other loss not caused by the Organizer, and acknowledge that the Organizer shall bear no liability for such matters.

I/we also understand that video recordings, photographs, names, bib numbers, and race results from this Event may be broadcast, displayed, sold, and published worldwide, including on the Association's website. I/we agree that the Organizer or any entity authorized by the Organizer may collect, process, and use the personal data provided by me/us, send information relating to road running events, and use my/our likeness and race results for promotional purposes.

I/we hereby exclusively authorize the Organizer and persons designated by the Organizer to take audiovisual recordings and photographs of me/us during this Event, and, within the scope necessary for organizing and conducting this Event, to collect, process, and use all personal data concerning me/us as provided for under the Personal Data Protection Act. I/we further agree to exclusively authorize the Organizer and persons designated by the Organizer to publicly disclose, use, and sell audiovisual recordings and photographs of me/us taken during this Event.

Any person or entity other than the aforementioned authorized parties is prohibited, without my/our consent, from photographing, filming, or otherwise recording me/us during this Event and subsequently using such recordings or photographs for sale or any commercial activity.

I/we further guarantee that valid identification documents and information will be provided for the purpose of verifying the identity of the participant or participating team. I/we confirm the foregoing statements and agree to assume the corresponding legal responsibilities.

Once registration has been completed, I/we shall not, for any reason, question or challenge any matters stipulated in the Competition Regulations.
Participants who have experienced unexplained chest discomfort (including chest pain or chest tightness), unexplained shortness of breath, or who have a family history of heart disease or diabetes are considered to be at high risk of myocardial infarction. Participants are advised to carefully consider their own safety and not to participate against their better judgment, or to consult a physician to assess the risks of participation and undergo an electrocardiogram (ECG) examination in advance. Participants are also advised to obtain additional personal accident insurance at their own discretion.

At the time of registration, please confirm that you are physically capable of completing the Event within the prescribed time limit.

Once the registration process has been completed, participants may not, for any reason, request a change of participant name, event category, clothing size, or a refund, nor may the registration qualification be transferred to another person because the participant is unable to participate.

Any person who has not completed the registration process shall not be eligible to participate in the Event. Pacing or accompanying registered runners without completing registration is strictly prohibited.

Personal Data Protection Notice

The Chinese Taipei Road Running Association (hereinafter referred to as the “Association”) is committed to protecting personal data. In accordance with Article 8 of the Personal Data Protection Act (hereinafter referred to as the “Personal Data Protection Act”), the Association hereby provides the following notice:
In accordance with the Personal Data Protection Act and other applicable laws and regulations, and within the scope of the specific purposes approved by the competent authorities, including consumer and customer management and services, contracts and matters relating to similar contracts or other legal relationships, marketing, other business operations and activities specified in the Association’s articles of association, and the organization and promotion of road running events, the Association may collect your personal data and the personal data of other relevant persons, including name, National Identification Number, date of birth, address, gender, nationality, telephone number, email address, emergency contact person, and the emergency contact person's address and telephone number.

With respect to the aforementioned personal data, the Association will only process or use such data within the scope and duration of activities for which the Association serves as the organizer, co-organizer, or event administrator, and within the territory of the Republic of China (Taiwan) and at the locations where the Organizer’s information storage equipment is located.

The personal data collected by the Association will, in principle, be provided in electronic or paper form to the Association, outsourced service providers, or third parties having a cooperative, commissioned, or other similar relationship with the Association for processing and use.

The Association retains the personal data you have provided. Pursuant to Article 3 of the Personal Data Protection Act, you may exercise the following rights with respect to your personal data:
1. You may inquire about or request access to your personal data.
2. You may request a copy of your personal data.
3. You may request supplementation or correction of your personal data.
4. You may request the cessation of the collection, processing, or use of your personal data.
5. You may request deletion of your personal data.
If you wish to exercise any of the above-mentioned rights, please contact the Association's service hotline at +886-2-2585-5659.

When the Association needs to collect, process, or use your personal data for the purposes stated above, you may freely choose whether to provide your personal data. If you choose not to provide your personal data, or if the information provided is incomplete, this may result in your inability to successfully register for the relevant marathon or road running event, or may prevent the Association from obtaining insurance coverage for you in connection with this Event.
If, due to the needs of the Event, you are required to collect the personal data of participants or other relevant persons and provide such data to the Association, please fully explain and inform the relevant persons, in accordance with this notice, of the matters concerning the protection of their personal data and their related rights.
I/we have carefully read and understood all of the above terms and conditions relating to the race event and registration procedures, and agree to fully comply with and abide by them.
